INSURANCE
In-Network Insurance Plans:
Anthem Blue Cross, Blue Shield of California, Cigna, HealthComp, Health Net, Interplan, Medicare, Medicare Railroad, St. Joseph/Providence Health Medical Network (IPA), Sutter Medical Group (IPA), Imperial Health Plan (IPA), TRICARE, UnitedHealthcare, Aetna, PacificCare, UMR, Great-West, Humana, and United Health Plan.
If your insurance plan is not listed above, please contact our office to confirm network participation before scheduling an appointment.
Please Note: If you have an HMO plan, prior authorization is required before scheduling an appointment with our office. Without prior authorization, your visit may not be considered a covered service by your insurance.
Examples of HMO plans requiring prior authorization include Sutter, St. Joseph/Providence Health Plan, and Imperial Health Plan.
WE DO NOT PARTICIPATE with Kaiser, Anthem Blue Cross EPO and All Pathway PPO's including EPO and EXPO. If you have any questions about our participation with your insurance plan, please call us or your insurance company before scheduling an appointment. If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor prior to your visit.
Appointment types
• Medical dermatology-regular office visit, covered through your medical insurance
• Telehealth – At this time, Dr. Julie Plante, Dr. Dale Westrom, and Dr. Susan Amaturo are offering in-person appointments only and do not currently provide telehealth visits.
• Cosmetic consultations are not covered by medical insurance. The consultation fee is $100 or $200. If you choose the $200 consultation, the fee will be applied toward the cost of your first treatment. The $100 consultation fee is not applied toward treatment. During your visit, transparent pricing for any recommended procedures will be discussed. Please note that procedure costs are separate from the consultation fee. In most cases, cosmetic services are scheduled after the inperson consultation.
• Surgical consults are medical office visits designed to evaluate the lesion under consideration. Note the procedure will be scheduled after the in person consult and aren't done the same day.
Registration
After scheduling an appointment, you will receive a registration link from eDerm. Login with the patient's last name and date of birth. If you didn't receive the email, please check your Spam Folder. If you are an established patient, you should see a "Welcome Back!" notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, you will need to call the office otherwise a duplicate chart will be created.
